Continuing Education Certificate of Completion Policy
Purpose
Premier Psychotherapy, LLC is committed to providing participants with timely and accurate documentation of successful completion of continuing education (CE) programs. This policy outlines the procedures for the issuance, distribution, and maintenance of Certificates of Completion.

Eligibility for a Certificate of Completion
Participants are eligible to receive a Certificate of Completion after meeting all applicable program requirements. Requirements may include, as applicable:

Full attendance for the required instructional time, or completion of the attendance requirements specified for the program;

Participation in required educational activities;

Completion of any required program evaluations;

Successful completion of any required post-tests or learning assessments for home study or other applicable programs; and
-
Compliance with all published program requirements.
Registration for a program alone does not qualify a participant for a Certificate of Completion.
Distribution of Certificates
Certificates of Completion will generally be distributed within ten (10) business days following verification that all program requirements have been successfully completed.

Certificates may be delivered electronically or in another format designated by Premier Psychotherapy.
If program requirements remain incomplete, certificates will not be issued until all required components have been satisfactorily completed, if permitted by the program.

Certificate Information
Certificates of Completion will include, as applicable:

Participant's name;

Title of the continuing education program;
-
Date(s) of program completion;
-
Number of continuing education hours awarded;
-
Delivery format (when applicable);
-
Name of the continuing education provider (Premier Psychotherapy, LLC);
-
Any required approval or provider information applicable to the continuing education program.
Corrections and Replacement Certificates
Participants who identify an error on their Certificate of Completion should notify Premier Psychotherapy as soon as possible. If the error resulted from Premier Psychotherapy, a corrected certificate will be issued at no cost.

Participants may request a replacement Certificate of Completion for previously completed programs during the organization's record retention period. One replacement certificate will be provided upon request. Additional replacement certificate requests are subject to a $10.00 administrative processing fee per certificate. Premier Psychotherapy reserves the right to waive this fee at its discretion.
Replacement certificates will be issued using the participant's original completion information and will not reflect changes to continuing education requirements that occurred after the original program date.
Record Retention
Premier Psychotherapy maintains records of participant completion and issued Certificates of Completion in accordance with its Continuing Education Record Retention Policy and applicable continuing education requirements.

This policy is reviewed periodically and updated as necessary to maintain compliance with applicable continuing education standards and organizational policies.
