Continuing Education Record Retention and Security Policy
Purpose
Premier Psychotherapy, LLC maintains a secure recordkeeping system to preserve the integrity of its continuing education (CE) programs, ensure compliance with applicable continuing education standards, and support participants who request documentation of program completion.
Record Retention
Premier Psychotherapy will maintain all continuing education records required by applicable continuing education provider standards for a minimum of five (5) years following the conclusion of each continuing education program.
Records maintained may include, as applicable:

Program agendas and schedules;

Learning objectives and educational materials;

Presenter qualifications, biographies, and credentials;

Conflict of interest disclosures;

Attendance verification records;

Participant registration records;

Certificates of Completion issued;

Program evaluations;

Learning assessments and post-test results, when applicable;

Promotional and marketing materials;

Complaint and dispute documentation;

Program revisions and administrative records necessary to demonstrate compliance with continuing education provider requirements.
Record Accuracy
Premier Psychotherapy makes reasonable efforts to ensure that continuing education records are complete, accurate, and current. If an administrative error is identified, corrections will be made promptly, and the date and nature of the correction will be documented when appropriate.
Secure Storage
Continuing education records are maintained using secure electronic and, when necessary, physical recordkeeping systems designed to protect the confidentiality, integrity, and availability of program records.
Electronic records are maintained using password-protected systems with access limited to authorized personnel. Physical records, when maintained, are stored in secured locations with restricted access.
Reasonable administrative, physical, and technical safeguards are utilized to protect records from unauthorized access, disclosure, alteration, loss, or destruction.
Confidentiality
Premier Psychotherapy is committed to protecting the privacy of participants. Personally identifiable information collected for continuing education purposes will be used solely for program administration, attendance verification, issuance of certificates, compliance with continuing education requirements, and other legitimate business purposes.
Participant information will not be disclosed to third parties except as required by law, to accrediting organizations during authorized compliance reviews, or with the participant's written authorization.
Access to Records
Access to continuing education records is limited to individuals whose responsibilities require such access for the administration, quality assurance, auditing, or compliance of continuing education programs.
Authorized personnel are expected to maintain the confidentiality and security of all participant records.
Backup and Recovery
Electronic records are routinely backed up using secure storage methods to reduce the risk of data loss resulting from equipment failure, accidental deletion, cybersecurity incidents, or other unforeseen events.
Premier Psychotherapy maintains reasonable safeguards to promote business continuity and the preservation of continuing education records.
Record Requests
Participants may request documentation of program completion during the record retention period. Premier Psychotherapy will make reasonable efforts to provide requested records in a timely manner in accordance with organizational policies.
Replacement Certificates of Completion are issued in accordance with the organization's Certificate of Completion Policy.
Audit Cooperation
Premier Psychotherapy will cooperate with authorized reviews or audits conducted by accrediting or regulatory organizations. Records necessary to verify compliance with continuing education provider requirements will be made available to authorized reviewers in accordance with applicable standards and legal requirements.
Record Disposal
At the conclusion of the required retention period, continuing education records may be securely destroyed in a manner that protects confidential information and prevents unauthorized access.
Electronic records will be securely deleted using reasonable data destruction methods, and physical records will be shredded or otherwise destroyed using secure disposal practices.
Policy Responsibility
The Continuing Education Administrator is responsible for overseeing compliance with this policy, ensuring records are maintained in accordance with applicable continuing education standards, and periodically reviewing recordkeeping practices to support ongoing quality improvement and regulatory compliance.
Policy Review
This policy is reviewed periodically and updated as necessary to maintain compliance with applicable laws, continuing education standards, accreditation requirements, and organizational practices.
